Q: With Nudge, Nudge, Wink, Wink can you bluff and draw a card, or do you have to prove you know “what is meant”?

A: Well, that all depends on how good a job of bluffing you do, doesn’t it? Unless some other player suspects you have no idea “what is meant” there’s no reason they should challenge you.

If you’ve seen the Monty Python sketch where this line is used, the whole joke is that the guy (Eric Idle) saying “Nudge, nudge! wink, wink! Know what I mean? Know what I mean?” actually has NO idea what he means. He’s trying to make sexual innuendos while being completely ignorant about sex.

So it’s entirely okay, nay, appropriate, even, that you’d take a card, because, like Eric Idle, you definitely know what is meant… right? Right.
